All 2017 American Legion & Junior American Legion Players should complete the 2017 Form 2 and bring it to the parent/player meeting. The form can be opened and printed by clicking this link.

Please complete all fields EXCEPT the School enrollment field on page 2 (this field will be completed by Post 757 Staff). Completed forms for both Senior and Junior teams should be completed and given to Coach Barr at the parent meeting along with a copy of your driver's license (or state identification or birth certificate).
